All airlines (although see note about TG) appear to give them departing from Bangkok, so the list below is mostly useful for inbound. It's important to remember that this is Thailand so just because an airline has given them out before is no guide they'll be given out in your case. It's worth asking both air-crew and the ground crew at the gate for them, as they're also occasionally forgotten. Thus the list below tends to reflect "this airline has given them out at least once" rather than being any guide to what will actually happen...

The list

AI (Air India) - J arrival and departures
AF (Air France) - J arrival (distributed in the jetway leaving the aircraft), J and W departures
AY (Finnair) - J arrival (jetway) and departure
BA (British Airways) - J arrival and departure (BE AWARE there have been instances where cabin crew have not handed out the pass and when asked either only given to the asking passenger or advised they aren't given out)
CX (Cathay Pacific) - OWE departure, J/F arrival and departure
EK (Emirates) - J and F arrival and departure. Gold and above in Y excluded by local rules.
EY (Etihad) inbound. Not sure of criteria.
HU (Hainan Air) - J outbound only, no inbound
HX (Hong Kong Airlines) - J departure and J arrival
JL (Japan Airways) - J/F arrival, departure J/F (OWE denied)
KE (Korean Air) - J arrival and departure, assume F as well
LX (Swiss Airlines) - J departure
LY (ElAl) - Arrival and Departure for J+F plus KD
MH (Malaysian Airlines) - J/OWE (not sure about OWS) outbound, J inbound & outbound
NH (ANA) - Outbound. Also J arrival (4/2016)
OS (Austrian) - J departure
OZ (Asiana) - J arrival and departure (no OZ F service in BKK) (no arrival 26/03/16)
PG (Bangkok Airways) - Blue Ribbon (C) or Premier ongoing for arrival and departure
QR (Qatar Airways) - J/F arrival and departure, OWS/E departure
RJ (Royal Jordanian) - J+OWR (and above) outbound, J inbound ,OWR/above in Y (if you manage to find RJ staff at the jet bridge)
SQ (Singapore Airlines) - J arrival and departure or PPS Club/Solitaire - sometimes on request only.
SU (Aeroflot) - J arrival and departure
TG (Thai Airways) - F+J have dedicated security area at airport for outbound, for arrivals not given but your boarding pass is sufficient
TK (Turkish Airlines) - J arrival
UL (Sri Lanka Air) - J+OWS arr + dep
VN (Vietnam Air) - J outbound only, no inbound
9W - J (Premiere) arrival and departure

Notes:

1) Immigration seems to only care about the airline stamp. Recent experience continues to be hassle free when the printed date and/or names do not match or are not filled in. YMMV. Edit: In early June (2015) was told by dragon to fill in the name field on the Fast Track voucher. She then checked to see if it matched BP name.

2) If you're inbound to BKK and have not received a voucher onboard the aircraft, be sure to ask!

3) Fast track vouchers do not have to be used on the date issued. If regular immigration queues are short on your arrival date, it's often worthwhile to pocket the voucher for a different arrival date when you may not have premium access (or to give as a gift to friends/family coming to visit).

I'm still at a loss to understand the relevance of that post within this thread.

Does KE give out fast track on arrival? I'm assuming on departure they do.

Unfortunately Druk Air does not give fast track passes. Would have come in handy yesterday. Around 5PM the passport control lines were all really long. And the 5 PM shift change made for even longer waits while the new officers got all of their stamps in order.
Something must have changed just a few weeks later. I flew Druk in business class mid-Nov 2010 and without asking was given a fast track pass upon check-in. On return to BKK there was an agent who met arriving passengers with pre-filled fast track passes. I was connecting in BKK, and they didn't have a pass for me. (I assume they knew this since I checked a bag through to SIN, and Druk in Paro was able to interline the bag with SQ, so there was no need to pass immigration in BKK.)

As another data point, no fast-track passes given in Emirates First on EK413 SYD-BKK on 11th December. I asked and they said they didn't have any...

Arrived just before Chrismas on LX F.
Several people with signs for F passengers at the jetway, escorted thru fasttrack.

A few days later arriving in F on TG. Got fasttrack card at the door of the plane, then person with name waiting for me inside the terminal building, escorted thru fast track, helping with luggage even putting it into the Taxi.

A few days ago arrived on qatar. They had no immigration cards on board, no fasttrack for the business passengers. Not impressed with the '5 star airline'

TG did not issue FastTrack cards last week on DPS-BKK, even though we asked for it. The FA's told us to just go to the FastTrack channel and present our BP's. We did that. The immigration officers wanted the passes. Mr. Megatop, who is Thai, managed to talk them into taking us, which was a good thing because FastTrack was empty and regular Immigration was a zoo. The officers told us that TG should have given us the passes and they were just being cheap to save the cost.

GA issued the passes to us for a recent outbound to CGK. CX issued them for my SIN departure earlier this week. Earlier this year, MH issued them inbound and outbound.

In the past, DL has issued them on request outbound, but the FastTrack channel is often not quite open when it's time to pass through for the early DL departure. I've never seen DL issue them inbound.

Question.

Last time I went through BKK was in TG J class and we were given fast track cards.

At Fast track they didn't ask for the cards, and were fine with the BPs, so we held on to the fast track cards.

I'm passing through BKK next week, but in Air Asia (cheap cheap deal). Can I try and use my TG fast track card, or no hope in hell?

I often use my TG fast track cards when arriving on PG. Just go for it.

Yeah we tried to use our EK F boarding passes at the FastTrack counter and they told us off and sent us back to the regular line. Welcome to Thailand! Thanks Emirates!

If your inbound airline does give out fast track passes and your outbound does not, try to snag an extra pass while inbound. We flew in on EK F and doubled up on fast track passes. Then, when we left on KL, we managed to use the extra EK passes. Thankfully they weren't paying close attention and we avoided a huge line.

Last july we flew back from BKK to FRA with Lufthansa in business class. At check inn we got a fast track voucher.

I have no idea if this is a regular "service", but at that time it was quite helpfull though we were late and there were long lanes at the regular counters for passport control

I just departed BKK on a CX economy ticket and was held up at the check-in counter for about 20 minutes because they delayed the flight 30 minutes and saw that it would make me miss my connection in HGK. So they resolved that in such a way that didn't affect my overall itinerary at all but presumably to make up for the prolonged check-in process they gave me a fast Track pass. even though it was still like 3 hours until departure, I was on an economy ticket, have no kind of OW status at all and wasn't actually inconvenienced in any way by their resolution of the connection issue. Just the prolonged check-in while they resolved it. Very nice of CX. Immigration looked very busy and I was happy to get to the lounge faster. Sorry to hear about folks on paid F tickets standing in line
