Read me first!
All airlines (although see note about TG) appear to give them departing from Bangkok, so the list below is mostly useful for inbound. It's important to remember that this is Thailand so just because an airline has given them out before is no guide they'll be given out in your case. It's worth asking both air-crew and the ground crew at the gate for them, as they're also occasionally forgotten. Thus the list below tends to reflect "this airline has given them out at least once" rather than being any guide to what will actually happen...

Notes:
1) Immigration seems to only care about the airline stamp. Recent experience continues to be hassle free when the printed date and/or names do not match or are not filled in. YMMV. Edit: In early June (2015) was told by dragon to fill in the name field on the Fast Track voucher. She then checked to see if it matched BP name.
2) If you're inbound to BKK and have not received a voucher onboard the aircraft, be sure to ask!
3) Fast track vouchers do not have to be used on the date issued. If regular immigration queues are short on your arrival date, it's often worthwhile to pocket the voucher for a different arrival date when you may not have premium access (or to give as a gift to friends/family coming to visit).

I arrived late last night on DL in DeltaOne (business class). FAs didn't offer fastback cards during the flight, but on impulse when deplaning, I generally asked the gang of local DL staff meeting the flight if they had fast track vouchers. One was handed to me without question. They may have seen my DM/2MM metal tag or might have guessed that I had been seated up front as I was one of the first few people to leave the aircraft, but I wasn't asked to show my boarding pass or otherwise questioned about my request. I suspect that this is YMMV.
BTW the airport was quiet at around 11 pm, although I didn't walk past the regular immigration lanes so I don't know whether there were long lines. I was almost tempted to pocket the fast track pass for later use, but I was walking with a DL employee who knew I had it and directly me to the fast track desks.
DL staff seemed to be escorting some passengers to their connections. In addition, a few people on my flight has apparently paid for their hotel's meet and great airport VIP service.

2. I presume I can use fast-track with my APEC card (use it for arrivals, so presume I can use it for departures).

Is it possible that one side was closed late in the evening, around 11-11:30 pm? Kettles from my flight seemed to be walking past where I went to the fast track lanes.

Both the east and west Arriving Passport Control halls are open 7/24/365.
The Fast Track "area" is directly between the east and west halls. There is a map on the back of the Fast Track cards. It is impossible to get to the Fast Track area without walking by one of these two halls - owing to the moving walkway, where it is relatively simple to gauge the queues.
Often there are are only one or two Immigration Officers working in the Fast Track area, and VIPs/Crew can trump ordinary folk, so the wait can sometimes greatly exceed that in one of the "regular" halls, where there might be a dozen IO's staffing desks. Unless I see the queue backed down the ramp to the regular halls I use them, and save the Fast Track coupons for others, or later use.
I always have checked bags so end up waiting ~ 10 minutes (at a minimum) anyway. Usually pick up a liter of spirits (duty free on arrival, in the baggage claim area), and book the AoT transpo while waiting.

We do have some very old TG premium cards (about 4 years old), wonder if they would work still (won't bother now, as have ABTC, but might be useful for family, etc).
