Here's an overview of cruise line smoking policies:

Where can you smoke on a cruise ship? Generally, you can light up in designated outdoor smoking zones, and sometimes in casinos, nightclubs, cigar bars and cabin balconies (depending on the cruise line).

Nonsmoking areas typically include cabins, restaurants and theaters.

Electronic cigarettes are usually treated as traditional cigarettes and may only be used in designated smoking areas. Some lines, however, allow passengers to use e-cigarettes in the privacy of their (otherwise nonsmoking) cabins.

The cruise lines with the most restrictive smoking policies are Oceania Cruises -- smoking is permitted in two small areas only -- and Italian cruise line MSC Cruises on its North American ship, Divina -- where smoking is permitted only in the Cigar Lounge and on the port side of designated outside decks. Even smoking in the casino is not allowed.

The least restrictive policies allow passengers to smoke on their cabin balconies. Costa Cruises, Fred. Olsen Cruise Lines and Saga allow balcony smoking.

Smoking on cruises is generally more prevalent on any ship that sails with a large European or Asian contingent, compared with a ship filled with North Americans

Since we just cruised on MSY Wind Surf:

Windstar Cruises

General: absolutely no smoking anywhere inside (including cabins) or French balconies on the motor vessels. Discarding lit smoking materials over the side can get you evicted at the next port (yes, I’ve witnessed this), but extinguished materials must be disposed of onboard.

Smoking materials used (cigarette, cigar, pipe, vape) not restricted by type.

Some evenings in good weather conditions, “Cigars under the stars” evenings are held by the Star Bar, aft upper deck.

MSY Wind Surf: from highest deck down.

Star Deck: Port side by Terrace Bar. A couple of outside tables, umbrellas available. This bar is open limited hours, but bar stewards will bring drinks to you. “Cigars under the stars” and “Cigars and whisky (maybe single malts, iirc) under the stars” offered at 9 pm some evenings.

Bridge Deck: by aft Pool Bar, port side. Several tables on that level, some shaded or with umbrellas.

Main Deck: aft by Compass Rose Bar, several tables, some umbrellas and some under cover.

On the three motor vessels the locations are similar, two aft port outside locations. One on the Star Deck near the Star Bar, one other iirc aft of the Lounge and less desirable.
