Holiday Inn Express hotels in Cape Town (central) and Durban (Umhlanga) offer smoking rooms.
I have stayed at the Umhlanga property and it is an excellent HIX - up there in terms of quality with most full service HIs.
I have not (yet) stayed at the CPT property, but reviews are good - I will report back after my 8 nights there in December.

Cape Town - Southern Sun 'The Cullinan' has smoking rooms on the 7th floor. Rooms featured in the CPT TRs in my signature.
Other hotels around the Western Cape we have used allow smoking on the balcony/terrace.
Stellenbosch - The Eendracht. Limited number of rooms.
Franschhoek - Franschhoek Country House and Villas. Almost every suite has this facility.
Hermanus - Misty Waves Boutique Hotel. Some rooms have balcony/terrace.
Other hotels around the Western Cape we have used allow smoking on the balcony/terrace.
