USA / Washington DC / Airports / IAD (Washington Dulles) [Now closed]
Terminal C, Gate C4
Very small, lousy ventilation and usually jammed before the time that flights to Europe are set to depart
Terminal D, Gate D30
Large, good ventilation and at the very far end of Terminal D
(it's a long walk if you have a flight in the C gates but if you have the time, it's better than the one at C4)
Terminal B, Gate B37 and Gate B73
I have not been to the one at Gate 73 but the one at gate B37 is small and can get a bit crowded around the times some LH and LX flights depart

I did not see a smoking lounge yesterday at the C gates, but did find the one a D30 and agree it is well ventilated and in a good location.
My only gripe is that I arrived at C6 and Departed from D21. I cannot believe how long a walk that was......
Perhaps I didn't see the one at C4 because I didn't pass it.

Yikes.... I swear I thought I was walking forever to get to that other gate. How do they expect people who might be elderly or disabled to go that far? I didn't even see the electric carts to move those people between gates like they do in EWR. Perhaps I was there too early (6am).
So the other smoking room is at the OTHER far end? Yikes.... that really is far to go. But at least they have them, of which I was grateful.

"Happy" user of the B-Gates smoking facilities, which happen to be well-located for a user of the BA Lounge facilities
The B37 is particular well-located for the BA l/h boarding gates [42/44] but it's a disgusting hell-hole these days. Gone are the days when you could press a bell to get waiter service from the Bar next door
The B73 is more civilised - just.
