Very messy.
You can always check your bags in to FRA only, that's okay. But flying
a different itinerary like this may cause you big trouble. I've seen
similar case in the other direction and it took very long time
to sort it out at check in *and* it might have incurred additional
charges because of different itenarary. It is pretty close
to rebooking on the spot.
A medical trick might not work, but is not necessary if you check
your bags to FRA only. There is a very good airport pharmacy
in FRA.
One of the posters said he let his bags fly alone, wondering
how *that* happened...

I reckon the only sane thing to do is to re-book FRA-IST at a later date, whatever the cost. In this case they should interline the bags. Pulling a medical can inconvenience a great number of people and is not the appropriate solution.
