Although not in Ireland, I had the same issue a while ago. Customer Services simply replaced the card and haven't had a problem since.

From my own experience: Yes from LHR; probably from WAW.
I’ve never had a problem from LHR, but the outstations are a bit random. WAW has been OK for most trips.
