Co-pay Upgrades

Are upgrades using miles and cash

1. Confirmed at time of booking (booking will be about 3 weeks before actual flight)?
2. Will I get miles for this upgraded flight?

Thanks

Has anyone used Bank Direct, American Beaver Funds, Guaranty Bank or TD Waterhouse specifically for earning AAdvantage miles?

Any comments on service and speed of posting miles?
