Easiest is to take Amtrak or some other method to re-position to Philly. PHL has flights to Rome and Venice (seasonal). The cheaper cash fares are out of JFK, though.

I was pricing PHL-FCO-PHL today and noticed extra taxes equate to roughly $75. I used to pay less than $30.

What gives?

UPDATE: Called AA and they confirmed I was not seeing things — the taxes...

I wouldn't advise flying into LHR with miles because the surcharges are absurd. Best bet is to fly into Milan or Rome and take the train - Trenitalia Frecciarossa or Italo are your best options.
