Buying an extra seat for yourself - fees, miles, upgrade chance?

In this thread about getting an extra seat the following reference was made,

Three questions:
Fees: For the booking I assume this means you have to pay the booking fee for calling as well as likely a fee for reserving seats, anybody have experience with that?

Miles credit:
For miles credit I know that united gives you bonus miles for your extra seat and with Lufthansa you earn both; but I found nothing so far for SAS.

Upgrades:
will this remove your chance for upgrade like with a special meal order?
Or the fact that you will be very late in the checkin order since you will be checking in at the airport?

And last: if you have a connecting flight and only the second flight has the extra seat (like f.e. with TATL), is it okay to check in at the first airport or does it require you to 'checkin twice'?
I assume not, I assume it is just for "go to the airport and let a person handle that"

Sas has this on their website under seat selection
BUY AN EXTRA SEAT
If you need an extra seat for more comfort or for medical reasons,* you can book the seat next to yours for a fee. Pre-order when you book through a travel agency or by contacting SAS Sales & Service**

*Extra seats may not be used to store extra baggage.
**If you’ve booked an extra seat, you’ll need to check in at the airport (not online).
