Insane indeed. I just booked this. On top of it, there is currently a double miles promo going on for travel in premium classes, so with 2x miles we get about 40k miles for this trip? Or something like that. Count the value of the miles against $1215, wow.

Thought I would answer this for you , The no show fee is if you dont turn up for the flight and then want to change it without changing before hand if that makes sense, So if you simply didnt turn up but then phoned them a few hours later you would be charged the no show fee, to simply dump the last segment is different and should be fine.

Does anyone have positioning recommendations that credit to AA? I've been researching high EQD-earning ex-USA itineraries (PE or above) but I haven't found anything sub-$2,000 roundtrip.

I booked a ticket but now I want to add a stopover on my return but it jacks up the price for the dates I want. If I do not show up for my flight for CMB but call them after my departure for a flight next day, do I have to pay the fare difference as well as the no show up fee? if only no show-up fee, it is cheaper to skip the last leg and fly next day after paying the no show up fee but I want to make sure....

I was able to price out 2 adults and 3 children under 12 for $5250 all in with the discount. That's crazy, $1050 per person roundtrip. This is cheaper than the old CMBSPL NW fare about 10 years ago and involves a far better seat. Too many cheap fares ex-EU and ex-Asia, and not enough time...
For the ask above about changes to add a stopover, the change fee is only $50 (quite reasonable) and the no show fee is much higher (either $250 or $500). I'd work with QR to try to make a change, it should be that the last segment is priced at the historical fare you have now and so only $50 is due, and if you try to do this online prior to departure it may not be correctly calculating. Again, I'd suggest trying to call QR by phone to work this out. BTW, many of the older dirt cheap QR and EY fares ex-CMB had strict notes in the fare rules about no stopovers and no ability to short check luggage to DOH. So the fact that this one allows a SO, even if short is pretty nice.
