The KY address still forwarded, although who knows when that stopped or stops.

I had a look and I indeed was registered as left the US. I also got an entry stamp to Canada on my leaving the US, so I should be fine (I hope .
One last question would you leave the slip in the passport or discard it?

I would keep someplace safe, but not in the passport...
