How's it looking, post Nov 2021?
 
The last post I see is late Nov 2021. My understanding is that Omicron is doing damage in Peru, but requirement is still vaccine OR PCR (72 hrs). Is that still the case?

We just booked flights to Lima 3/26-4/6 and plan on spending the whole trip in Huaraz, just wondering if anyone on the ground in Peru can comment on the situation since the latest post was 2 months ago.

Basically the difference from the changes in november 2021 is that negative PCR test validity is for results within 48 hours from embarkation.
You can still enter with just the full vaccination certificate (1 or 2 doses) + Health declaration form

For domestic flights, booster shots for those over 50 will also be required (begins on Sunday 1/23)

Update: For domestic flights, vaccine booster shots for those over 40 will also be required (either that or <48h negative PCR test).

(booster shots for those over 40 is also the new requirement to enter most indoor businesses)

I heard the ban on South Africa (or travellers who had been to SA in the last 14 days) had been lifted. I was pointed at Supreeme Decree 010-2022-PCM anyone confirm?

That is correct, even though there's still a decree that bans passenger flights directly from SA until the end of February (kind of pointless because there aren't any).
In any case, there isn't a specific restriction for travelers that have been in SA anymore.

Update - starting April 1st
 

4.4 Los peruanos, extranjeros residentes y extranjeros no residentes de 12 años a más cuyo destino final sea el territorio nacional, en calidad de pasajeros e independientemente del país de procedencia, deben acreditar haberse aplicado la primera y segunda dosis de vacunación contra la COVID-19 en el Perú o en el extranjero, y la tercera dosis los mayores de 18 años que residan en el país y se encuentren habilitados para recibirla, según protocolo vigente. En su defecto, pueden presentar una prueba molecular negativa con fecha de resultado no mayor a 48 horas antes de abordar en su punto de origen. Los menores de 12 años solo requieren estar asintomáticos para abordar. Aquellas personas que muestren síntomas al arribar a territorio nacional ingresan a aislamiento obligatorio, según regulaciones sobre la materia.La Autoridad Sanitaria Nacional se encuentra facultada para la toma de pruebas diagnósticas para la COVID-19 a los pasajeros que arriben al país, estableciendo las medidas sanitarias complementarias para los casos positivos.

4.5 Los pasajeros del servicio de transporte aéreo nacional mayores de 12 años residentes y no residentes, solo pueden abordar si acreditan haberse aplicado la primera y segunda dosis de vacunación contra la COVID-19 en el Perú o en el extranjero, y la tercera dosis los mayores de 18 años que residan en el país y se encuentren habilitados para recibirla, según protocolo vigente. En caso no hayan completado las dosis requeridas de acuerdo a su edad, deben presentar una prueba molecular negativa con fecha de resultado no mayor a 48 horas antes de abordar. Los menores de 12 años sólo requieren estar asintomáticos para abordar.

Main difference: Booster required for adults that are residents in Peru and eligible to receive it. (Otherwise, PCR required as before).
