Including both CPT and ADD in DONE4
 
Starting JNB or CPT and would like ADD as second African destination which appears available via DOH.

DONE4 continent routing is Africa>Europe>NA>Asia, Is it possible to go Africa>Europe>Africa>Europe if the Doha stop is transit only? CPT-DOH-ADD-DOH-XXX

If possible, does the DOH-ADD count as 1 Europe segment?

No, it is not possible
The rule you run up against is this:
4(e) Only one intercontinental departure and one intercontinental arrival permitted in each continent except as follows:
1. Two permitted in North America.
2. Two permitted in Asia when one is for travel between the Southwest Pacific and Europe/Middle East.
3. Two permitted in Europe/Middle East for travel to/from/via Africa.
If travel is to/from Europe in both directions, itinerary may not include Mauritius/South Africa.
So you are allowed only one departure from Africa and one arrival

You could end your rtw with ...DOH-ADD
but then you would have no way to get back to southern Africa on the ticket

Thanks. So it's possible to CPT-Europe-NA-ASIA with the final 2 segments being HKG-DOH-ADD? The DOH-ADD would count towards a Europe segment?

Yes it is possible to do that
The DOH-ADD segment will be an inter-continental segment, between Europe/Middle East and Africa

For completeness, the rule on where you can start and end a Oneworld Explorer is:
4(c) Travel may originate at any point for which fares are published and must terminate at the same point, except that origin‐destination surface segments are permitted as follows:
a. within the country of origin
b. within the Middle East
c. between the United States and Canada
d. between HKG and China
e. between Malaysia and SIN
f. within Africa
g. between Maldives and Sri Lanka/India
Since your proposed itinerary starts in Africa (at CPT) , it can end anywhere in Africa

You could consider buying JNB-ADD separately. Usually relatively cheap in economy, only around 5 hours.It depends where you are positioning from/to after the RTW.
