What does 'one per household' mean?
 
Can my Mom and Dad each sign up separately for a Fidelity account to receive separate bonuses? How exactly is 'one per household' defined, what if the husband and wife are separated or don't hold joint accounts?

AFAIK "one per household" means one per address. I could see this working if one person signed up with home address and the 2nd person used a work address or something.
