For JFK, I have changed recently from NYC yellow cabs to Cloverdale (Big Apple) Car (718-236-1000). They can meet you in terminal but since they always have a car or two out at the airport, if you call them as your foot hits the ground, they will have a car for you by the time you walk to the door of the terminal.
Since I (and most other NYers) only use JFK for international travel, their meeting point at the new International Arrivals Building is across the first street, on the island walking to the right near the yellow cab line.
My recent change is due to the medallion taxis being so uncomfortable for anything more than a few minute's ride. In the Fords (which are now virtually the only cars that are used), the way the seats are situated and the shield behind the driver (in front of the passenger) makes you feel as if you are sitting in a hole and has made this one of the most unattractive modes of transportation on the planet (possibly surpassed only by the NYC subway).

I love exective limo. The owner is an american woman and the cars and limos are clean and comfortable. They will even meet you inside and generally on time. All the drivers seem to be american or british. 800-716-2799. I hate standing in the taxi line for 30 or more minutes and then having an unpleasant ride with drivers who barely speak the language.

Oh, how I beg to differ. My airport of choice for frequent trips to the city and Long Island from St. Louis is JFK. Way, way more convenient than LGA (well, ok, unless I'm heading to the UES when it might be better to go to LGA. But that's only if conditions are perfect at the airport itself.) Once the train over the Van Wyck is complete (by the way driving to LGA this morning noticed only about 3 more spans left to go before the whole thing is bridged, move all that equipment out of the median) it will be even better.

And as for those taxi seats, they absolutely have to be that low down. Can you imagine sitting in the back of a cab heading uptown on 1st Avenue doing 60 MPH to make all the lights and as you catch air crossing intersections, if that seat weren't basically on the floorpan of the car you'd end up with a concussion. Oye veh!
