HC fixed my miles!
 

Just received a parade of HC envelopes in the mail today each containing only one cert for AA. These were replacements for the 5k on UA they sent me in error. And I did send the 5k on UA back to them.

I'm trying to find out where I can contact HC to determine where my voucher/miles are. I'd submitted for 1,000 miles back in early June and have seen anything....thanks!

Get the 800 phone number from the HC website to inquire; and/or send an email to their customer service from their web site as well.

You can use one award request certificate to request as many multiples of 500 mile certificates as you want, correct? So all I need is one Healtychoice certificate plus all my UPCs to get as many mile certs as I qualify for.

The wording on the certificate I got off the back of one of the meals is somewhat vague.

dg1-- yes, you can use one form to request as many mile certs as you want, from multiple airlines. ie: I requested 5k miles on AA and 5k on UA (to help out a fellow FTer) but HC sent me all 10k on UA.
