What is the $250-minimum companion pass? Is it something that can be traded between FTers, so I ought to look for one through Coupon Connection?

US Airways issues credit card holders companion passes that let you fly a companion with you for $99. The fare must be at least $250 and be within the U.S., I believe. I don't think they're transferable, unfortunately, as the pass-holder must be traveling on the PNR.

Is the minimum $250 the all in price or tax excluded?

Just base fare, though for this particular flight, there are lots of options at around $320 that put base fare right at $260ish. These companion tickets do earn miles just like regular fares, so it comes out to be roughly $220pp (tax incl) for two full mileage earning tickets.

Obviously no great shakes unless you're trying to burn one of these price-limited coupons. It is still common to find transcon mileage run fares on the Star Alliance for about $200 out of BWI. One of the better "predictable" domestic airline deals still around.
