I have to add one thing: I was charged for breakfast via room service in the past, even as it was included. It's a not uncommon practice, sometimes included breakfast can't be taken through room service (understandable if a buffet is served at a restaurant). Nevertheless some programs like FHR state, that you can choose between free breakfast in the restaurant or vis room service. So with a FHR-booking no charges apply. This means it can be that someone with a B&B-rate pays an extra charge while a person with a FHR-rate don't.
