I stayed at Hotel Copernicus when I was there a couple of years back. It's a member of Relais & Chateaux. Standards were met in my opinion :-)

I stayed at the Hotel Stary this spring - it was overall pretty good. Great location; great rooftop bar; room was large but bed was basically at floor level.

I stayed at the Copernicus a couple of years back, we had a great stay. Quaint room with fresco paintings dating to the 16th century. Excellent food and drink. Not the height of luxury but I don't know if you'll find anything better in Krakow.
