KL questionnaire
 

KL occasionally hands out questionnaires at the gate and at other locations. I've just completed one which asked me for my level of membership in the Flying Dutchman program. It also asked me how I had checked in. The list of options included every conceivable way of doing so (including curbside), save one : online.

The fact that the form was printed three years ago (it says "KL GROUND 06/03 in small print) explains the questions, but not why these questionnaires are still used.
