Mosaic Qualifiying Segments Reset on Jan 1?
Say I earn points in Oct/Nov/Dec 2018 but not enough to hit Mosaic, do I start from scratch Jan 1, 2019?

Yes, unfortunately the program is based on a calendar year, so you effectively start from scratch on Jan 1 from the perspective of Mosaic qualification.
