1. Have not been to Singapore and Kuala Lampur so I don't know if the time there is OK or not; two days in Tokyo is very little; not sure what you like to do but there are tons of stuff to do. You really need to some searching before you ask such vague questions.
2. I stay at the Keio Plaza Hotel; I like Shinjuku and the hotel; It is about the same price as a hotel in Paris or London; Simple meals are inexpensive and about the same price as any large city; you can always buy some ready meals in the many convenience shops
3. Maybe you should consider a tour of the city. This is a reputable company that I have used several times. http://www.jtb-sunrisetours.jp/
4. Weather in November is cool with possibility of rain; I would check with weather.com a few days before you leave.
5. English is not widely spoken but most important signs are in English and with pictures and plastic model food you can figure it out.
6. Go to amazon.com and look for this book - buy it: Tokyo (EYEWITNESS TRAVEL GUIDE) by DK PUBLISHING (Nov 15, 2010)
