If all on 1 ticket, and depending on certain variables even on multiple tickets, you'll be transiting in London and go through Immigration at MAD and customs in SVQ.

Here is a link for connecting at LHR, assuming you're flying in and out of LHR, http://www.heathrow.com/flight-connections

4 hours at LHR is too much in my opinion as is 1 hour in MAD too little. Try to get on the previous MAD flight.

As above would YYZ-LHR-MAD-SVQ be on 1 ticket/PNR? Be careful if purchased from an on line travel agent
Which airlines? Airlines have minimum connection times MCT.

In any case you will need to check your eligibility to transit LHR (visa-passport) and onwards. People with some passports require a UK transit visa
