Please take a moment to answer a short questionnaire on repeat visitations to resorts to help my research project.

University? Department/major/program? Degree?

Based on the questions and the fact that the topic of the survey is barely addressed in the questions, I'm guessing it's a liberal arts program at the university of Phoenix.
If you want to know if people have made repeat visits to resorts, don't ask if people have visited a museum or a "specific city". If you're interested in repeat visits, why does it matter if it was a group visit or if I'd recommend it to others?
Quite frankly, I don't see how these questions in any way can help you make any conclusions regarding repeat visits to resorts.
I don't think anyone should waste their time taking the survey.

This survey disregards EVERY common required practice for the protection of participants in human subjects research, and your dissertation committee is engaging in unethical research practices.

At least follow the basic ethical concept of 'Respect for Persons' <www.hhs.gov/ohrp/humansubjects/guidance/belmont.html#xrespect>. Have some consideration for others, instead of the selfish "data uber alles!"
