UAFF1 Award - United Companion Ticket

Has anyone ever used this award - UAFF1 - for 40,000 points you get a companion ticket?

Is there a limit as to the number of companions? I would like to take my wife and my daughter as companions. Secondly, does the United ticket have to be in a certain fare class?
