Thanks I changed my reservation to show the AAA Web CDP, which increased the rental cost by about $60. Do we both have to be present to pick up the car (she is arriving a week later ...)? I have asked her to sign up for a Gold membership so they will have her license and info on record anyway.

You will have to go back to (the/a) location to have her added as a driver. You can't do it in absentia, particularly as she has a different address. Also, as she will be a first time Gold member, she will need to present proof of ID (driving license) before she is confirmed in the system.

Also, the knowledge of each agent about the AAA additional driver for free rule varies wildly. For example, in Vegas, I spent 45 minutes explaining it and finally got them to call their own 800 number to verify that it's true. Quite a training and systems issue. Bring print outs (even though they sometimes refuse to believe them).

When I went to pick up the car at ZRH and I asked they were not friendly at all and just said that the AAA/CAA offer is good for North America only. What a rip-off as I had changed my 20% CDP to a 10% AAA CDP, which is not insignificant for a 4-week rental. I will send a couple of complaining emails for sure.

What are you complaining about?
If you read the T&Cs of the AAA CDP, you can clearly read this:
Benefits valid only in US and Canada and do not apply on international, Advantage or Package and Book Together and Save rentals.
Basically that CDP is not valid in ZRH or anywhere else outside the US and Canada.

I agree with UAPremExecflyer. This is your fault. I posted a link to the AAA benefits for you and in the first paragraph it says the benefits are only available in the United States and Canada. You never gave us the exact rental location even after being asked by roadman3313. If you had posted your particular existing rental reservation was outside of the United States or Canada, we would told you CAA benefits, which you posted you were thinking of joining, would not apply.

Is there anyway to get the Amex Plt time extension and the free second driver? I (we) have AARP, AAA (which from the above posts, won't work with my Amex CDP), can join USAA. Suggestions?

It all has to do with the CDP on your contract. So, no if you're going to use your AMEX CDP (not unless you're renting in a place like California which does not allow second driver fees).

Thanks. Renting at ORD so I guess it is a no go for the freebie. Is there a way to add the 2nd driver in advance who is also a GP member? I assume I can call about this?

You're going to have to do it at ORD or stop in at another Hertz location after the rental starts. You can't add the second driver in advance. This isn't like a hotel reservation.

Unless the second driver is your spouse (and only if the rental originates in California or Iowa for a spouse) or a co-employee (only with that business CDP), ALL OTHER additional authorized operators MUST be present to be added to the contract.

i'm renting a car tomorrow at LAX and will be picking up from the gold canopy...i'll need to add my fiancee on as a driver, which i know in CA should be able to be done without any additional fees. can they do this at the gate as i'm driving out, or will i need to stop at a counter and talk to someone?

You need to go to the Gold Booth to verify ID for new paperwork.
Can I suggest you try doing a search ... this topic has been asked and answered myriad times.

i did search and read that thread, i didn't see anything in there about what specifically i had to do once i got there to add her on if i was renting from the gold canopy (which is what my question was). i saw a bunch of conversation about going to other locations or doing it at the counter...my question wasn't about "how to get a free second driver" as i'm renting in CA and already have that covered.

You will need to go to the booth. In my experience, both you and the her will need to sign for the extra driver (even if you're gold).
