Changes to the CD Terms and Conditions in early January 2019 confirm the following:

Effective 1 March 2019, new Chauffeur-drive service reservations are not available in conjunction with Skywards Business and First Class Classic Rewards and Economy to Business Class Upgrades.

That part of the website has been dumbed down considerably recently and for many (most?) destinations that information is no longer there.
But you are right in that for LAX it is still there. $28.75 per 10 miles, excluding tolls outside the 50 miles complimentary radius.
Including gratuities, it says. How that will in reality pan out in the USA remains to be seen......

Did exactly this and it worked out great. Told emirates I would be having 6 bags. Called the limo company out of curiosity, they said it likely would be a 14 person mini bus. Not exactly luxury but fine with me. Their website however shows a stretch limo option, and that is what was sent. Everything fit and driver didn't mind doing two drop offs. Thanks.

Sorry if this has been covered before but
Are there distance restrictions (ie too short) at Dubai for Chauffeur drop at an airport hotel such as Meridian Hotel & Conference Centre?

No, that's my usual place to stay. Never had an issue with drop off or pick up.
