Set your expectations to zero. Upgrades are typically based on status and you just happen to be flying a very elite heavy sector. And while I am sorry that you have an injured leg, be assured that the airlines have all those stories (real or not) before so a sympathy upgrade is also unlikely.

Why don't you buy 6000 miles (ie US$228) - you'll then have enough to upgrade the LHR-DXB sector (ie 25,000) and if you use ExpertFlyer, you will be able to check upgrade availability. (PS I just checked and there is ONE award seat available)
