From the T&C:
Upgrade Priority within the same Medallion tier and fare class is applicable only to Basic Card Member and within the Complimentary Medallion Upgrade Program
So any primary cardholder of any Medallion status will have a higher upgrade priority of any other non-cardholder of the same status and fare class.

It's just that PMs are not often in a position on the upgrade list for the tie-breaker to make a difference.

That would depend on the route. There are some where the upgrades typically get to PMs and not GMs.

And the footnote:

So no effect on RUC's, GUC's, and mileage UG's.

A PM with a Reserve card on the same fare class as a PM without the Reserve card will be 17th on the upgrade list and the PM without it will be 18th.

Yep. It's likely to be most useful for those who frequently find themselves on the bubble for an upgrade.

Whether the Reserve card still makes a difference is debatable. Some published sources say no, some yes. In any case, it only counts for free elite upgrades, not for priority when using instruments.
