Website says refundable, Diamond Desk wants me to pay $500 more. Looking at M fares MNL-BOS for 11/6 and 11/7. M is approx $1500, Y= $2000. The website clearly says the M is changeable and refundable. The DM desk says that the M is changeable for $250 and not refundable.

I have found the DL agents just assume all non-Y fares are nonrefundable. I had to insist that the agent look closer at the fare rules once and after being on hold for 10 minutes, she realized her mistake. In other words, the DL agents are wrong if it is listed as refundable on the website (of course, minus the cancellation fee).

OK, the website does say that "a fee may be applied" right after it says cancelable/refundable.

Even if the OP used one way fares, if they're on the same itinerary, the total change fee for one passenger should be only $250 (one fee for changing the ticket), not $500.

The Y fare was $500 more than the OW M fare. The Y fare is completely refundable, the M fare is refundable minus $250.
