Question about Lounge Access

Does Skyteam Elite Plus have lounge access if flying from continental US to San Juan (SJU), PR?

No, that is considered a domestic itinerary.

Plus it's in the Caribbean, which I guess means it's doubly disqualified (the
lounge access rules specifically excludes the Caribbean from definition of "international" for
the purposes of lounge access).

And I doubt that there is a Sky Club at SJU.

There used to be a nice Delta Crown Room at SJU. It was closed as part of the CRC downsizing in Summer, 2008, before the lounges were renamed SkyClubs:

Don't need a lounge. There is a very relaxing McDonald's right next to the USDA scanners pre security.

I really hate that airport. Only redeeming factor is that SJU was an easy upgrade and sometimes they would swap 757's with AVOD for the run down 757-200s that normally run the route, or you could catch a 767 with AVOD around the holidays.

I started carrying an inflatable seat cushion because the seats in the 757-200s that we normally got were so bad.
