A 70,000 points offer was offered back in March 2017 and expired March 31st, 2017. EXPIRED
The requirements for the application process varies from bank to bank based on various online data points collected from FlyerTalk, Doctor of Credit, etc.
Some banks require that you go into a branch and apply personally. Other banks want you to have another account open for 90 days before you can get this card. Some banks (if you already have a relationship with them) let you apply via mail. Sometimes you have to be a resident of the state they operate in, while some were able to get the card without living in those states. It is very much YMMV. You will need proof of income (W2, check stubs, etc). The only states City National Bank operates in are: NY, DE (only trust office, no go), DC, GA, TN, MN, NV & CA. The bank is looking for high net worth individuals, but that's not exactly consistent either.
The card has no foreign transactions fees and is fee-free the first year, then $400 annually.
Register your card for online access (cno.cnb.com). This changed on Sept 19 2019.

Question about Airline incidentals- it does state that it covers fees and TSA Pre - my husband and I are planning on getting global entry- can we get global entry for our 2 kids that would be covered under the $250 airline incidental?

Testing AA 4x$250 today
Testing 4x$250 AA e-Gifts today. Should work fine as has for years. Will report back when credit hits.
Also working on MS via grocery for net 2.9 cents per dollar spent for cruise ships i.e. each point 3x and yields 1.3 cents for sailings when you spend them.

Random ........ discovered today. City National Bank has a partnership with Shell, BP, and a few other gas companies to offer discounts per gallon at the fuel pump, using City National Rewards. If an option, the fuel pump will ask you if you want to redeem points to get 50 cents off per gallon. It won't say anything about City National Rewards, nor how many points it will take to redeem them. I said yes to this offer last month because I thought I was redeeming my SHELL FUEL REWARDS. Apparently I wasn't. I was redeeming 2,000 City National Rewards. News to me. I just spoke with a rep at City National Rewards who says she can't reverse the redemption and she can't turn it off to prevent it in the future. This is an absolutely offensive and ........ partnership. Nobody should accidentally redeem credit card points at a fuel pump. Completely ridiculous. I'm not using this card at gas stations ever again and I no longer feel bad about the amount of travel reimbursements I've gotten.

My suggestion would be to use the $250 incidentals first then use the card for the global entry payment. Remember to use a different AU card for each GE payment. I know they'll use your incidental credit for airline lounge membership first before reimbursing you up to $550 for those that put more than $50k spend, not sure if it works the same with GE payments.

Delta GC's still do not work
I tried three different times buying Delta GC's, b/c i'm an idiot and didn't think to check here first. They all posted as DELTA AIR GIFT CARD, even though i bought them through the FlyerTalk approved link in the Delta Amex reimbursement thread. Using the same link for my AmEx cards gave me DELTA AIR ATLANTA and i was reimbursed. So continue to stay away from Delta GC's for this card.

Shell Fuel Rewards
I experienced the same scenario of accepting 50 cents off per gallon with no idea this was 2,000 CNB points. You can avoid this by disabling Shell's new "redeem rewards & pay for fuel in a single step" feature. You will have to revert back to either swiping your fuel rewards card or entering your alternate telephone ID, but you will avoid the link to CNB rewards redemption.

btw, at my local Costco gas, my Crystal codes up properly as gas for 3x.
Some other cards with gas bonuses don't code up properly on the Costco ones.
If you are a Costco member, it's worth testing out at your local pump.
Normally we always used our Citi Costco card - since its just one swipe - but I'd rather have the spend/interchange go to CNB rather Citi.

You can redeem the CNB points all year round, while the Costco rebate only comes once a year, and basically has to be used in store.
Plus if you want to hit the 50k CNB lounge benefit, every little bit helps, assuming one is only using organic spending.
But that double swipe - one for the membership and the second for the payment card - will probably deter my SO from doing this.
