The requirements for the application process varies from bank to bank based on various online data points collected from FlyerTalk, Doctor of Credit, etc.
Some banks require that you go into a branch and apply personally. Other banks want you to have another account open for 90 days before you can get this card. Some banks (if you already have a relationship with them) let you apply via mail. Sometimes you have to be a resident of the state they operate in, while some were able to get the card without living in those states. It is very much YMMV. You will need proof of income (W2, check stubs, etc). The only states City National Bank operates in are: NY, DE (only trust office, no go), DC, GA, TN, MN, NV & CA. The bank is looking for high net worth individuals, but that's not exactly consistent either.
The card has no foreign transactions fees and is fee-free the first year, then $400 annually.
Register your card for online access (cno.cnb.com). This changed on Sept 19 2019.

Data points on this thread show some gcs still reimbursed, is it known PGGM doesn't work any more?

Some replies in this thread in October stated that WN PGGM is still working, but the tax portion must be as low as $60. I don't see any further information. Does anyone know more about it recently?

I can confirm that WN PGGM still works as of 12/14 where I received the travel rebate for an award booking on 12/7.

Datapoints on downgrade/Close account after fee was posted
Just my datapoints
1. On 30 Nov the fee was posted
11/30/2016 ANNUAL FEE $400.00
2. I actually thought it would be a couple weeks later and had not called yet. I called on or about 12 Dec
3. I explained I didn't think I would be traveling as much and didn't feel the card fee was going to be worth it this coming year.
4. CSR offered to downgrade my account to a No Fee card. I said sure. And that I would like to have the annual fee removed then also. CSR stated he could not authorize that but would check on that and forward the request to change the account. Got my phone number and said I would be contacted.
5. Called today to check on status as I had not heard anything from anyone. The CSR I spoke to said actually it looks like the Fee was REMOVED/Credited back. I had not even looked online as I thought they would be contacting me.
6 Thanked her and she also confirmed that my card (and an AU card) was being shipped already.
7 I then checked online and sure enough, on 13 Dec a 400 credit was posted.
12/13/2016 ANNUAL FEE ADJUSTMENT $400.00
So this is what I have seen.
We have another account (and AU) that will likely post the end of Dec or so and I will plan to simply close that account before the fee posts.
I don't have a lot of confidence in what remains and also the fact that the Delta PGGM no longer works.
thanks
YMMV

I like Clear, would like free extension with the card if possible.
Edit: actually, when I logged into my CLEARME account, I see "renewal amount $0.00 for Jan 22 2017". Does that mean I'll be getting free CLearme? That would be amazing!!
