The requirements for the application process varies from bank to bank based on various online data points collected from FlyerTalk, Doctor of Credit, etc.
Some banks require that you go into a branch and apply personally. Other banks want you to have another account open for 90 days before you can get this card. Some banks (if you already have a relationship with them) let you apply via mail. Sometimes you have to be a resident of the state they operate in, while some were able to get the card without living in those states. It is very much YMMV. You will need proof of income (W2, check stubs, etc). The only states City National Bank operates in are: NY, DE (only trust office, no go), DC, GA, TN, MN, NV & CA. The bank is looking for high net worth individuals, but that's not exactly consistent either.
The card has no foreign transactions fees and is fee-free the first year, then $400 annually.
Register your card for online access (cno.cnb.com). This changed on Sept 19 2019.

Most of the time the AA eGC receive the travel credit no problem....but there are plenty of data points out there that occasionally it doesn't work especially if one buys a cluster on one day. Given that the President of CNB cards is manually approving AF refunds (according to oldreddit) they could certainly task someone with eyeballing air incidentals credits to see if they meet the terms & conditions. This is one reason why I would be circumspect of thinking people will be able to fully utilize the credits in 2020 -- I don't think it will be the same rules on the back end.

How long does it take to remove authorized users? Just trying to make sure that they can do it before the year ends in case they charge for them at the beginning of next year.

I called the number on the back of the card. Requested to cancel all AU cards. Agent said she will contact my banker. Next day I checked online and did not see them so I assumed it was done...much faster than I expected. But I will check again in the last few days of the year just to make sure...because ya just never know with this bank!

When I was emailing back and forth with my relationship manager a few weeks ago she removed 2 of my AUs and it was effective immediately.
