The requirements for the application process varies from bank to bank based on various online data points collected from FlyerTalk, Doctor of Credit, etc.
Some banks require that you go into a branch and apply personally. Other banks want you to have another account open for 90 days before you can get this card. Some banks (if you already have a relationship with them) let you apply via mail. Sometimes you have to be a resident of the state they operate in, while some were able to get the card without living in those states. It is very much YMMV. You will need proof of income (W2, check stubs, etc). The only states City National Bank operates in are: NY, DE (only trust office, no go), DC, GA, TN, MN, NV & CA. The bank is looking for high net worth individuals, but that's not exactly consistent either.
The card has no foreign transactions fees and is fee-free the first year, then $400 annually.
Register your card for online access (cno.cnb.com). This changed on Sept 19 2019.

It's all YMMV. When I added the AU's 2 years ago, two of them were aged 6 and 8 at the time. My banker had no issue with their ages, and in fact help me set a $500 spending limit on their cards (not like they've ever physically possessed them anyway). However, a colleague of mine went to do the same thing and add his kids, and was told 18 by his banker (and the other banker he tried on another day) -- go figure.

Crystal Visa Infinite Card accounts opened on and after January 16, 2018. With this offer, qualifying clients can earn 50,000 Bonus Points when they spend $5,000 or more within 90 days after account opening.

Crystal Visa Infinite Bonus Points Offer Disclosure
- 1 The 50,000 Bonus Points offer is available to clients who do not currently have, and have not had within the 24 months prior to the date of application, a City National Bank Visa personal credit card account. The Bonus Points will be awarded subject to the conditions below. The Crystal Visa Infinite Credit Card is subject to credit approval. If you apply and are approved for a Crystal Visa Infinite Credit Card account, to qualify and receive 50,000 Bonus Points, you must make net new purchases (purchases less returns & credits) totaling $5,000 or more, which are posted to your Crystal Visa Infinite Credit Card account, as applicable, during the first 90 days following the date of account opening. Please allow at least seven (7) days for processing your application request. If additional information is needed, the processing of your application may take longer and delay the opening of your account. The posting date for a purchase may be later than the transaction date, depending on the date merchant information is received by City National Bank. Therefore, if a purchase takes place near the end of the 90-day period, the posting date may not occur until after the expiration date of the 90-day period, in which case the purchase will not be counted toward the $5,000 or more spending requirement. Bonus Points are earned on a per account basis and not on each card issued under an account. PIN-based and ATM transactions do not earn Points. Taxes on the Bonus Points, if any, are the responsibility of the Crystal Visa Infinite Cardholder. After qualifying, please allow 4 to 6 weeks for the Bonus Points to be credited to your City National Rewards account. To be eligible for this Bonus Points offer, the Crystal Visa Infinite Card account must be open and in good standing (such as, not delinquent or otherwise in default) at the time the Bonus Points are posted to your City National Rewards account. See the City National Rewards Program Terms, Conditions and Program Rules and FAQs for further information about earning Points. Program Rules are subject to change at any time without

It just means that individual reservations need to be booked instead one a single reservation with multiple passengers. I had to cancel my original reservation and rebook to get it under the dollar threshold. Let's hope it comes through in a few weeks.

I talked to the VP and she said there is no end date. However it seems like the first year annual fee won't be waived with the promotion. PM me if you want to directly talk to the VP.
