What should a letter to close a credit card account include?

One sentence, "would you kindly close credit card #XXXX-XXXX-XXXX-XXXX" in my name (and other(s) if applicable).

Also, check T&C of your particular credit card to determine whether those T&C require you to do anything else such as cut the card in half and return it with the request.

a phone call will suffice. and while on the phone, you can ask them any questions you want....
