I'm sure that fare class could have a lot to do with it. I do remember that I was on a K fare that one time I was charged the $75 (but not the fare difference). I do travel on mostly discounted fares though, and rarely on Y or H, and yes on the changes I have made I have not been on a changeable fare. I guess I should just consider myself lucky and be glad. Regarding whether or not they mentioned the fee and/or the waiver, I have had it happen different ways. I have had them tell me up front that they wouldn't have to charge me, I have had them tell me they would have to charge me and I have asked to have it waived (some have had to go to supervisor), and they have at times not said anything at all.
If it were strictly revenue based, I am not sure I would score a lot of points, though, as I travel about 80-90k a year, all domestic, and mostly discounted fares.
