Earn bonus miles based on purchased class, or class I'm upgraded into?

Earn bonus miles based on purchased class, or class I'm upgraded into?
Never watched my statement for this, and now am confused:
If I book and use a Regional to upgrade into R at the time of booking, do I earn the bonus for traveling in First - or is my earned mileage based on my purchased fare only (plus elite bonus)?
What about booking in an Economy fare, then getting upgraded to PN or JN, are the miles earned still based on booked class only?

Does Upgraded R Fare earn miles on USAir?
I bank my Star Alliance miles with USAirways.
I have upcoming flight on UA ticket (also UA metal) that were purchased as W and V fare. UA SWU was used to upgrade to "R fare".
According to US Airways mileage earning chart for UA, R fare gets no mileage credit. I thought mileage earned follows purchased fare class?
