Asia Miles have been frankly useless, resorting to computer says no because they have changed the scheme so longer can do anything with upgrades. I have spoken to British Airways who have advised that if CX/AM contact BA through the OneWorld helpline they are happy to facilitate the change but I can not convince any agent to at least try this method.

Can anyone suggest a method by which I can reach an agent that might be able to think outside the box?

Carrying the old passport together with current one should work. Worst case would be to pay a fee to get the name changed at the airport.

I do not know the nationality of the pax. But many countries keep the maiden name in addition to the married name.
If it is a UK passport and the maiden has fully disappeared from the passport, then you are in a bind. I would assume that you would lose the upgrade. But the process to reinstate the "old" Y booking (issued on 125 stock) will not be straightforward as your ticket is now 160. And you will have to contact BA to make the name change on that Y BA booking as explained in the BA link above.

Some good news to close the loop. A very helpful agent from the BA Social Media team managed to use a very small schedule to take control of the CX tickets and reissue as BA on alternative flights. They were then able to change the name from their end.
All sorted - thanks for all the input here!
