We're going back a bit here, but my vote for worst is ANE (Angers), sadly dropped from the network a couple of years ago. Basically a cupboard with a DFS sofa in it...
Edit: I've found CWS's review (of course he's done one): https://www.flyertalk.com/forum/24927773-post238.html
Including "rubbish bin" in the entertainment and business facilities gives you an idea...

Best: LHR T3 CX or QF (I even prefer them to the Concorde Room).
Worst OW: Agree with OP on Milan Linate.
Worst Any: Kabul (there wasn’t a lounge and the toilets resembled those in the Trainspotting film, except they were holes in the floor).

BA:
Best - MAN, loads of space and great views.
Worst - T5 Galleries North, the only time i've ever left the lounge and gone to a restaurant in the terminal.
Others:
Best - CX Pier F.
Worst - CMB, i forget what its called. A few uncomfortable chairs arranged in the corner of the terminal with a privacy fence erected around them.

I love the idea of the “Oddly Newcastle” lounge name.
PS
@OP : BA lounge, or any lounge ?

I seem to remember the one in LOS being fairly grim. But then again, I had food poisoning at the time, so I might be projecting here!
For a CX lounge, the one at PEK is pretty disappointing.
I think we're pretty spoilt for choice at LHR T3 and at HKG. Some great options.

My experience is somehow limited, but so far the least appealing BA lounge is BRU. I dislike lounges with external restrooms, the view is nothing exceptional and drinks & food are very basic.
The best... maybe LHR GF in T5 (aside from the bathrooms).
Regarding the best OW lounge, there are CX in LHR & HKG, and AA flagship in JFK and MIA.
The worst... I think AA Admiral Club(s).

I'm not sure about the worst lounge overall but two of the worst lounge experiences I've had recently, have been at SJC and MCO. Coincidentally, I believe they are operated by the same operator.
For me what absolutely makes a lounge experience horrible is how packed they allow it to be. On every occasion visiting the MCO and SJC lounges, there has barely ever been any space to sit down! Food and drink options have never been that great either.
I've been a big fan of the CX lounge at terminal 3 and I know this may be divisive but I actually quite like the QR business lounge in Doha too.
Other than that, most of the lounges I visit kind of merge into one in my mind.

I agree with the space and views comment but the quality of the food isn't great and the gents toilets are in a very bad condition. MAN isn't even close to the top of my list.
Best lounge for me is CX The Pier. Worst lounge I've been to is probably SVQ.
