As already mentioned and trust me this works simply PM BAMissingAvios and they will sort this out for you in a few days with no hassle.
Send them your Locator reference for the booking, your BA Membership number and the flight details and that should be enough.
I have completed several claims including a couple of really tricky ones and they have sorted the issue out every time.
One issue the BA Gold Line could not resolve despite two months and several calls which was resolved by BAMissingAvios in a few days.
