Let's see:

The American Club - Upgrade to Immersion Suite from standard room. Further upgrade to Presidential Suite for a very nominal $200. F&B credit a $100 deduction from the bill, breakfast technically only in restaurant but I had no problem ordering room service.

Ritz Carlton Georgetown: Upgrade to one-bedroom suite confirmed prior to arrival. Same deal with F&B and breakfast. Service, as usual, was exemplary.

Went right after Sandy and beginning of Noreaster so somewhat unique circumstances in my booking.

FHR had cheapest rates on a Fabulous Room.

-No room upgrade (Suites sold out online)
-$100 F&B at BlueFin restaurant
-Breakfast amenity (Most basic continental b'fast ever)
-Late Checkout (Not offered or used)

Booked Deluxe King Guestroom Nov 21-23 (Thanksgiving Staycation)

In addition to the regular perks ($100 spa credit for the room, upgrade when available, noon check-in and 4p checkout), we also got -
- $150 food credit which we used for an awesome meal in Sixteen (special on FHR upon booking)
- Double upgrade to a 1500 sf Deluxe Suite

We had an incredible time and were treated very,very well. Room rate was $355 per night + tax

$100 resort credit
Full breakfast at main restaurant daily
Late checkout offered and used

FHR explained at check-in and given the details in writing. Had a great stay and the staff here is spectacular.

Booked two Superior Rooms at $189 per night over the Thanksgiving weekend. The public areas in this hotel are a bit tired and the service is lacking but both rooms were upgraded in advance to very nice Deluxe Suites.

Great buffet breakfast or american breakfast by room service included. You have to present a certificate to the server. Interestingly, the certificate says it covers gratuity, which is not normally the case in my experience.

Extra $100 per room in F&B credit went a long way over the weekend.

All in all a great stay given the price and location.

Booked Superior room - upgraded to Executive Suite
$85 dinner credit at Quattro
Full breakfast at Quattro daily
4pm late checkout offered and used

An excellent stay in a great downtown location. Rate was $159 + tax.

Trump Towers (NY). Booked executive suite. No upgrade.
Venetian Hotel Las Vegas. Booked superior room. No upgrade.
Mandarin Oriental SF. Booked corner room. No upgrade.

Recent non-US experience:
Raffles Singapore: Booked suite. Upgraded to better class suite.
Ritz Carlton Singapore: Booked suite. No upgrade.
Peninsula Bangkok: Booked suite. No upgrade.
Four Seasons Marunouchi Tokyo: Booked regular room. No upgrade.
Four Seasons Hong Kong. Booked club room. No upgrade.
Grand Hotel Cap Ferrat (France): Booked superior room. No upgrade.
Claridge's London: Booked regular room. 2 class upgrade to an amazing suite (3 times)
George V, Paris: Booked suite. Upgraded suite.

It is excellent value at weekends in particular. Quattro does not have great reviews on Yelp etc but we found food & service for dinner and breakfast were very good.

Ayana Resort and Spa (Bali, Indonesia)

Booked 1-bedroom Ocean View Villa, upgraded to 1-bedroom Ocean Front Villa.
According to the front desk, the 1 category upgrade through FHR is limited to rooms only (does not include villas). Amex Plat travel agent did not mention such restrictions when I inquire about the property.

Other Benefits/Amenities:
Late check-out till 4 pm
Breakfast at Dava Restaurant (a la carte menu, already included when you book a villa)
50-minutes massage for 2 persons, turned out to be Balinese Deep Tissue Massage.

Received the standard contracted benefits (breakfast, late checkout and $ 100 food and beverage credit), but no room upgrade of any description. Their basic category of suite was sold out, but previous posts suggested an upgrade to a penthouse room (essentially their standard room, but on the top floor, reserved for SPG plats) was a possibility. In the event, even this modest upgrade didn't materialise.

What I did get, however, was access to the nightly complimentary wine tasting, which I don't think is a contracted Amex benefit, and normally available only when booking higher category rooms.

Once again, this experience confirms that this is a hotel that gives room upgrades very infrequently.

Taj Campton San Francisco

One night at the Taj Campton in San Francisco. We paid $225 for a California Room, and were upgraded to a Deluxe.

Despite mixed reviews on TripAdvisor, we were very impressed with the service at this hotel. Breakfast in the restaurant was excellent. I stole quite a bit of my son's french toast. The casual eating area was packed at lunch, so we opted for room service which was...good enough. Glad we weren't actually shelling out $25 for a burger.

$100 food credit + $50 credit for breakfast. Early check in no problem, and late check out of 4PM was offered when we registered. No other amenities, although we turned down the turn down service, and perhaps missed something there.

The rooms are fairly small, but the hotel is right on Union Square and the price was excellent. We'll make it an annual staycation if the property remains available.

booked deluxe king, upgraded to executive suite.

breakfast is $56 credit each day - good for whatever you want.

$100 food/bev credit as well.

4pm checkout offered and accepted.

best of all, third night is free with current promo.
