Issue with new Amex Hilton Business app

Hi All,
My spouse applied for a new Amex Hilton Business credit card as sole proprietor. She got an instant approval.
The issue is that I got a notification from my credit monitoring service that Amex pulled my credit report. Amex did not pulled her credit report.
At first I thought may be some kind of mixup due to cookies or auto filled text on the web browser that I used, so I went to Amex website to check the application status. The approved app is shown under her SSN, not mine. So the correct SSN was entered in the application.
How could this happen? I don't really care who they pulled the credit from, I only worry that since I already had the card in the past, she will not get the bonus if Amex based it on my SSN.
What should I do now? Thanks

Have you looked at the credit bureau information directly to verify that the alert is accurate?

Yes, I did. It shows an inquiry from Amex on the app date.

Apparently, I am not the only person with this issue.
I signed up for the Amex Green Card but Amex opened it under my mother's name
