Has anybody noted the customer service seems to be able to do less than they used to. Constantly transfer you to other departments>

How do you get US customer service when in the US? Do you call the US Centurion number?

Jay66 is asking about using local Centurion customer service in the country you are visiting, rather than calling back home for help. In your example that would have meant contacting American Express Italy with a USA-issued card.
