Found this one on my Platinum card today:

(can be credited three times)

Get 400 additional Membership Rewards points by using your enrolled Card to spend a minimum of $40+ in one or more eligible transactions at a U.S. gas station by 11/1/2019. Limit of 1,200 Membership Rewards points. See terms for exclusions

I tried searching the forum for this but couldn't find it.

If I have an offer to spend $75 and receive $25 back, If I spend $60 and add $15, a total of $75, will I receive the offer? Or does it have to be $75 before tips?

In my experience it's whatever is the final, TOTAL line-item that's billed to your Amex - including tax/tip/delivery/fees.

as a broad rule, the terms don't mean squat.
the only amount that matters is the total amount posted to your AMEX statement.
the only exception i can think of is that somehow AMEX was able to tell a Sam's Club membership charge separate from Sam's Club purchases.
so, just follow the broad rule.

It's the net final charge, so if for instance a restaurant showed an initial pending charge of meal and tax as $68.50, and you added a $10 tip, don't worry.

Does anybody know if the Amex offers are still triggered by FHR? It says only valid when booking on the website (Kimpton in this case), but if I stay through FHR and pay with the same amex will it trigger it?
Debating whether I increase the room rate so I hit the target.

As long as you select "pay at hotel" when you make the FHR booking, you're good. When the charge hits your statement it will be from the property directly, and this is sufficient to trigger any offers assuming the normal spend requirements are met. Keep in mind that you'll forfeit the 5x MR if you go this route since the 5x MR incentive is only valid for "pay now" FHR bookings.

Great (I booked through an agent), now I will choose another room type to get it past the threshold

Did anyone receive a recent one for paying your cell phone bill? I 'swear' I saw it show up on my phone, and when I returned to the website later (having not added it immediately for some reason) it has disappeared. I didn't think offers did that.

It's a nuisance offer, so no surprise one would not rush to add it.
There are cards offering 5X on phone charges all the time. Currently Discover card also offers $20 for paying the phone bill twice.

One man's trash I guess. 1x (or 1.2x if Everyday) plus 500 would be 5% back for me.

But can you share the Discover Card promo? PM if needed since this is the Amex forum.

Insurance bill, get 20% back as statement credit up to $85. Valid til 11/5. Found on my BBP.
