I stayed at the Delano over Memorial Day Weekend.
When I checked in, the lobby was empty. The person at my front desk did not tell me about my benefits. I had a dinner to get to so i didn't come down and talk to them about that until the next day. No upgrade offered, but on that weekend i wasn't expecting one.
Overall I liked the hotel and would stay here again.

I stayed at the Delano last week for two nights. I can confirm that I got THREE days of free breakfasts @ $60 per day, or $180 total, as they include your check-in and check-out day. I was told explicitly at check-in that I got three days of credit for my two night stay. Good for $60 off room service per day (can order any item off menu, 24 hours/day) or for breakfast or lunch at Della's Kitchen (closes at 2pm). I was told that I could not combine the $60/day breakfast credit, and that it had to be used each day. This $60/day breakfast credit is IN ADDITION to the $100 resort credit that was also part of the package. Room service sucked, by the way; they claimed they were using Mandalay's room service due to some sort of transition.
The room price via the FHR web site dropped after my initial booking ($175/night to $150/night), and I easily re-booked at the lower rate and cancelled the original reservation. Prices dropped again quite significantly (to $100/night) two days prior to my stay and right after my cancellation deadline; I called but they wouldn't rebook at the lower rate. I suggest checking just before your cancellation window closes and re-booking if you can get a better rate.
I was upgraded without asking, and I believe the upgrade simply involved getting a strip view.
Someone asked about shows - I was able to purchase the 3x$57 (three attractions [e.g., dolphin interaction at Mirage] for $57) tickets and used my $100 credit for that, purchased AFTER check-in at the Maldalay Box office. Below are the terms of the $100 resort credit, directly from the letter I was given at check-in; notice it says "admission" as one of the options.
"Resort credit is applicable to the following DELANO LAS VEGAS and Mandalay Bay owned and managed outlets for Food and Beverage, admission, services/treatments, NOT applicable to merchandise or product: 3940 Coffee Bar, Aureole, Baccarat Lounge, Bayside Buffet, Beach Casino Bar, Beach Club, Beach Grille, Bikini Bar, Della's Kitchen, Eye Candy Lounge, Fleur, Franklin, In Room Dining, Lobby Bar, Mizuya Lounge, Mizuya Sushi, Moorea Bar, Noodle Shop, ONE Oasis Bar, Orchid Coffee Bar, Orchid Lounge, Raffles Cafe, RIVEA, Room Service, SeaBreeze Ice Cream, Showroom Bar, Skyfall Lounge, Sports Book Grill, Starbucks, StripSteak, Tiki Bar, Turf Club Bar, and BATHHOUSE Spa treatments and services only, not applicable toward merchandise or product. The last time I tried I ended up booking the 2-bedroom suite on an Mlife rate anyway without the FHR benefits, and I will say that if you have a group traveling to Vegas, the Delano 2-bedroom suite is one of the best deals in town, IMO. It's essentially just a regular Delano suite with a connecting door to another bedroom, but that makes for a nice and spacious setup with a total of 2.5 bathrooms and 3 beds -- 2 queen and 1 king. Rates are usually in the $400-500 neighborhood. (It is often possible to book two separate rooms for a bit cheaper, but that's not quite as much fun.)
All of the other two-bedroom suite options that I've ever been able to find at the top Vegas properties tend to cost twice as much, so Delano is uniquely valuable here. (Other properties' two-bedroom suites also tend to be 1 king bed in each bedroom, which may be better or worse depending on the configuration of people you're traveling with.)

Does anyone have any data points whether the 4th night free at Mandarin Oriental is the literal 4th night, or an average nightly rate for the stay?

I have a new MGM Amex Offer on my account for $50 off a $250 room charge through 10/5. Worth checking if others have the same.
Still not sure if FHR booking would work, though I suspect it might since the "book direct" language may be more about excluding bookings made through Priceline, Orbitz, etc., which would appear on your bill as charges from those entities and thus wouldn't get picked up by Amex's system.

One way to do this is leveraging cross-property charging, if you end up spending at MGM properties during your stay. I was in Aria Sky Suites and just charged Aria/Bellagio F&B and spa to my folio. Then at checkout I had the front desk run a charge of $300 against my Plat (I told them I had a credit to use up) and the balance against my JPMR Visa (prefer UR over MR) -- no complaints and the Amex offer triggered.
