Different passenger - Duplicate reservation error

Booked a domestic mileage ticket for myself and my SO (same reservation/PNR) a few days ago. We are trying to book another ticket on the same flight for her dad and get the following error

Something went wrong

This reservation appears to be a duplicate of an existing reservation which prevents us from continuing. Members: Review your Flight Reservations from the View Reservations link in Reservations.
I have tried with my account (original reservation booking account) and my SO's account. Both error out.

Any idea why it would be erroring out for a different passenger?

Trying making the reservation NOT logged in to AA.com and when in incognito mode (if using Chrome).

Sorry - I didn't clarify that we're trying to buy a mileage ticket for her dad as well. (its only 8.5k miles )

put it on hold with a different pax name, call in to get the name changed and ticketed

I'm going to call AA to get it ticketed shortly, but was quite confused as to what was tripping me up. Pax 1 and 2 are ticketed with miles using pax 1's miles. Pax 3 cannot be ticketed/placed on hold with Pax 1 or Pax 2's miles.

My friend, you know the answer already, since you're usually the first to identify it -- bad IT, such that the website can't distnguish between a reservation made for one passenger and that being made for another on the same flight. Wishing you a short and painless phone call to get it resolved!

Same thing when I travel with my son. We have different middle names, different DOB, different global entry numbers and unique Advantage numbers and AA still can’t figure out it’s NOT a duplicate reservation.

I'm telling you to make up a name to get the reservation on hold, try Fuzzy the Bear.

But what he's saying is that the website won't let him make a second reservation for the same flight, period, no matter whose name is on it (including Fuzzy's).

I suspected as much, but wasn't sure if I was missing some rule that I wasn't aware of. Haven't tried this specific situation before,

In this case, she has different everything. One part of the last name is the same, but that's it.

Tried that... still no luck. There's literally zero commonality.

Update

Called AA and got it sorted within 5 minutes. Tried a bunch of things to get one on hold, including Cyril Figgis, Burt Reynolds and Outlaw Country (Woo!)

I guess the system must stop multiple award bookings on the same flight, no matter who the passengers are. Maybe an anti-fraud thing, or more likely, bad coding.

Something funky is going on with AA's systems. The last few days, AA.com has been erroneously saying that previously cancelled tickets are still in fact valid tickets that have not been cancelled thereby preventing customers from holding/buying what the system considers to be a duplicate reservation. I haven't found any workaround other than to call AA. I can't seem to get AA.com to recognize a ticket has in fact been cancelled.

Indeed something funky going on. Expert Flyer has been unable to pull up AA flight info last couple days.

I tend to try to do the same thing. I spent an hour trying to change a reservation online, but it wouldn't let me. I called AA, the automated system recognized my phone number, and they answered immediately. It took the agent less than 5 minutes to get everything done, including my too-length praise for her skills.

BOTTOME LINE: It is best to call in. Do not spend precious time trying to "trick" the system into doing something.
