For travel using AAdvantage miles on other airlines, you must call reservations (you only pay the applicable "optional" fee for if you can book online but choose to call).

No major taxes when you fly through LHR. Only when you originate there. What you're seeing is primarily BA's fuel surcharges.

In this case we need to be vigilant to check ITA to make sure the taxes are charged correctly. It is NOT the first time AA system messes up the calculation of taxes. Recall it had erroneously charged CX YQ on award a few months ago. If I recall correctly, over the past 12 months, the miscalculation of taxes have happened periodically every few months. It is as if the IT dept did not understand that there is difference in selling partner tickets versus booking partner award with AA.

It may be because IB does not show up on BA itself either - I have found BA shows IB flights as BA code-shares when looking for award redemption. At least for the Intra-Europe flights I am searching for awards, BA site would show BA flight numbers then noted flight operated by IB. It wasn't like that last year.

That's because actual IB flights no longer exist on those routes -- they have all been converted to BA pseudo-codeshares:

When flying through LHR if you originate outside the UK there is no Air Passenger Duty (APD) levied. This is a UK government tax levied on departures only (ie, not on arrivals) and non-cheapest cabin (aka First, Business, Premium Economy) are charged twice the tax as cheapest cabin. The UK government has been increasing this tax at a rate that far exceeds inflation, and they have announced their intention to continue this. The amount of APD also varies by distance to destination (capitol city) and for long haul can approach $300 for premium class pax.

In addition there are passenger facility service charges levied by the airport. Those are charged for arrivals/departures/connections. LHR has service charges that are significantly higher than other major world airports. Connecting through LHR will almost always be more expensive than connecting through any other airport.

Some governments do pay more attention. CX for example I believe needs HKG gov approval for their YQ charges, and have had to reduce them from time-to-time (have any other airlines ever reduced them?)

What I find so shocking is that these started out as a surcharge... something supposed to account for a temporary spike in fuel costs. I don't think there is anything temporary about these surcharges (nor the cost of fuel) any more. As such I think there should be some form of regulation around these surcharges. For example, a airline could have complete freedom to impose a surcharge and reduce them... but if they wanted to increase the charge then the prior amount must roll into the base fare and the new surcharge becomes the amount of the increase required. This would more accurately reflect what is going on with long-term fuel cost trends. And if fuel really does drop again, then competition will force down the base fares again.
