In part: Air Marshals were on board, law enforcement sources said.

American Airlines spokeswoman Andrea Huguely said the Boeing 767 was not in any danger.

"The captain and crew assessed the situation and they did not feel as though there was a security issue on board," she said.

"People have walked off the plane normally," she said.

I don't get it. I thought that bathrooms can be opened from the outside by flight attendants.

The Wall Street Journal is reporting that the FAM did not break cover. "One official said a federal air marshal was aboard the plane but the situation was not so dire that the marshal felt the need to identify himself and take action."

I've seen the FAs jab the doors open using a pen or fork from the galley when the lock was stuck. It's a privacy lock, not a security lock.

Probably just someone got their wires crossed at Fox News (not that hard to believe!). Three adults couldn't even fit in a lav, for one thing.

I waited about 5 mins for a lav on a 763 from MIA to LAX once. I noticed that the "occupied" wording was slightly misaligned, and finally the FA decided to give it a shot.

Turned out the last pax to use the lav had never fully engaged the lock when they used it, and they apparently walked out without touching the bolt (remember, it was never locked). So the indicator said "occupied" but the lav was empty.

What is wrong with people...security is going to be very high today so act appropriately...don't give anyone any reason to suspect you of anything.

I seem to remember several occasions where I've see the lav doors locked by fa's and later reopened (before landing?).
There must be a way of getting entrance (without breaking down the door) if i.e., a pax passes out in the toilet.
But the news story, that there were 3 pax in the toilet at the same time seems a bit odd--maybe they were trying for some Guinness Book World Record?

when flying row 9 on the 757, I've seen FAs lock the door to the 2L lav for takeoff. When I asked why, their response was that they lock the door so that people don't enter the lav thinking it's door 2L in an emergency. Their surveys showed that with limited visibility, people will enter the first open door, and locking the lav made sure that people didn't clog the lav when they should evacuate the plane.

To lock the door from the outside, they used a ballpoint pen in the white plastic "Occupied" placard that has a little pinhole in it, sliding the mechanism closed. It would be just as easy to unlock from the outside.

The whole story isn't out yet, I suspect. But the pilots did not feel it was an emergency, the FAM(s) kept their identity hidden, and three adults in a 767 lav? Maybe an A380 handicapped lav, but really?

Turns out it was all a bit of 9/11 paranoia. Some people are WAY too jumpy about perfectly normal behavior:

Two fighter jets escorted a Los Angeles-to-New York American Airlines flight after three passengers' repeated trips to the bathroom aroused suspicion Sunday, the 10th anniversary of the Sept. 11 attacks, officials said.
"Oh my God! That guy pees too much! CALL THE PENTAGON!!!"
